مشكلة في رفع ملف xml إلى قاعدة البيانات

السلام عليكم ورحمة الله وبركاته

إخواني حاولت حتى تعبت من عملية تحميل ملف xml إلى قاعدة البيانات باستخدام خاصية Dom وهي مفعلة من السيرفر.

أرجو النظر في الكود ومعرفة المشكلة:
